Guidelines for Accessibility Resources
Student Responsibilities in Applying for and Receiving Reasonable Accommodations for a Disability
1. Request services by filling out a Accessibility Resources Application with Accessibility Resources. APPLICATION FORM
2. Provide Accessibility Resources with documentation of disability and recommendations made by the testing specialist. A Release of Information may be required by the agency. FORMS PAGE / DOCUMENTATION REQUEST FORM
3. When Accessibility Resources receives the documentation and recommendations required, the student and staff then work together to identify appropriate accommodations. FORMS PAGE / STUDENT RESPONSIBILITIES
4. If you are eligible, the Accessibility Resources Office then provides the reasonable accommodations. Communication with faculty is the responsibility of both Accessibility Resources staff and the student.
5. Please note: The appropriate forms must be filled out and signed each semester to continue receiving services.
General Guidelines Regarding Accommodations
1. Accommodations support abilities.
2. All students must comply with classroom assignments, projects, and tests as presented by the instructor and/or syllabus.
3. Accommodations provide appropriate supports, not substitutions, for the successful completion of assignments and tests and DO NOT take the place of regular classroom attendance.
4. M State complies fully with the provisions for the Americans with Disabilities Act of 1990, 42 U.S.C. section 12101, which prohibits discrimination in employment and public educational services on the basis of an individual's disability. An individual with a disability is one who has or is regarded as having a physical or mental impairment, which substantially limits one or more of that individual's major life activities.
Services cease at the end of each semester and must be requested at the beginning of each semester.
Personal Care Attendant Guidelines
M State does not provide Personal Care Attendant (PCA) services and is not responsible or liable for any consequences resulting from a student's association with a PCA.
Responsibilities of student assisted by a Personal Care Attendant
PCAs are NOT classroom assistants. Because a PCA is not a classroom assistant, personal care attendants are not allowed to assist with any academic tasks in the classroom or other academic settings. The need for a PCA in the classroom for non-academic tasks (such as monitoring a medical condition) will be determined byAccessibility Services on an individual basis.
Expectations of a Personal Care Attendant
· Follow all applicable college policies, rules, regulations, and procedures
· Assist the student before and after class but wait outside the classroom (unless deemed appropriate by the Accessibility Services office)
· Allow the student to take responsibility for his/her own progress or behavior
· Refrain from contact with or asking questions of faculty, staff, or others on behalf of the student
· Refrain from intervening in conversations between the student and faculty, staff or other students
· Refrain from discussing any confidential information about the student with faculty, staff, or students
· Follow the college's PCA guidelines
Should the PCA fail to follow M State policies and procedures, M State reserves the right to make a determination that the PCA will no longer be permitted on campus. It is the student's responsibility to secure the services of another PCA in the event a PCA becomes unable to perform services for a student.
