General guidelines for providing documentation of disability

Students requesting accommodation from Accessibility Resources must provide recent written documentation of a disability, with "recent" generally defined as within three years or less.

This documentation must be from a professional credentialed in the area of disability and should include a formal diagnosis, a summary of any appropriate diagnostic testing and a description of academic limitations specifically related to the disability of the diagnosis. Finally, this documentation of a disability must indicate that the severity of the diagnosis reaches the level of disability.

Individualized Education Plan (IEP) or 504

Generally these documents do not contain the information necessary to receive accommodations through Accessibility Services unless they contain the required information described in the guidelines above.

Prior accommodations at other institutions

Documents submitted to M State Accessibility Resources will be reviewed to determine eligibility. Prior accommodations by institutions other than M State do not guarantee eligibility for accommodations through Accessibility Resources at M State.
