Change of Address/Phone/Advisor Form

If your contact information changes while you're enrolled at M State, it's your responsibility to notify us to ensure our records are correct.

When changes occur:

  1. Submit the online Change of Address/Phone/Advisor form, or
  2. Update your address information under Account Management in SpartanNet
